چکیده

KEY LEARNING POINTS MS is defined as a single or multiple black/brown pigmentation bands arranged lengthwise along the nail unit. Physiological MS is more commonly seen in darker phototype individuals and is quite unusual in fair-skinned ethnic groups. MS is the result of increased melanin production by nail matrix melanocytes, which produces a visible pigmentation band in the nail plate. Distinction should be made between melanocytic activation (increased melanin synthesis with a normal number of melanocytes) and melanocytic hyperplasia (increased melanin synthesis with an increased number of melanocytes). The latter condition includes melanoma, which is the most concerning (albeit infrequent) cause of melanonychia in children. Multiple-band MS (as in our patient) makes melanocytic naevus and melanoma less likely as a cause. Our patient’s persistently good clinical condition since the appearance of MS (3 years ago) allowed us to exclude the rare systemic causes of multiple-band MS (eg, Cushing syndrome, hyperthyroidism, malnutrition, vitamin B12 deficiency, scleroderma and systemic lupus erythematosus inter alia).

Melanonychia striata secondary to pigmented nail matrix fibroma simulating nodular melanoma Melanoníquia estriada secundária a fibroma da matriz ungueal pigmentado simulando melanoma nodular

Melanonychia corresponds to color patterns in the nail plate, ranging from brown to black. It is a diagnostic challenge due to the fact it has several differential diagnoses, including benign and malignant entities. There are no reports of pigmented fibroma of the nail matrix causing striata (or longitudinal) melanonychia. In light of this fact, the authors report a case of melanonychia striata...

A Case of Longitudinal Melanonychia Secondary to Hydroxyurea

Melanonychia commonly appears as a longitudinal band, starting from the matrix and extending to the tip of the nail plate. Less often it presents a transverse band or may inolve whole nail plate. Cutaneous side effects of hydroxyurea are described in 13% of patients including increased pigmentation, hyperkeratosis, skin atrophy, xerosis, lichenoid eruptions, palmar and plantar keratoderma, cuta...

The one-nail brown band: macro- and micro-morphology.

The nail matrix contains melanocytes in the epidermal basal layer, but pigment is not visible in the gross anatomy of the nail plate in Caucasians. Hyperpigmentation due to an increase in melanogenesis, and subsequently melanin pigment, in the nail may be induced by certain drugs, tumours or endocrinological diseases (1-5). The whole nail may be affected, whilst some drugs or tumours produce lo...

Tangential Biopsy Thickness versus Lesion Depth in Longitudinal Melanonychia: A Pilot Study

Longitudinal melanonychia can be caused by melanocyte activation (hypermelanosis) or proliferation (lentigo, nevus or melanoma). Histopathologic examination is mandatory for suspicious cases of melanomas. Tangential biopsy of the matrix is an elegant technique avoiding nail plate dystrophy, but it was unknown whether the depth of the sample obtained by this method is adequate for histopathologi...

Key point in dermoscopic differentiation between early nail apparatus melanoma and benign longitudinal melanonychia.

Longitudinal melanonychia presents in various conditions including neoplastic and reactive disorders. It is much more frequently seen in non-Caucasians than Caucasians. While most cases of nail apparatus melanoma start as longitudinal melanonychia, melanocytic nevi of the nail apparatus also typically accompany longitudinal melanonychia. Identifying the suspicious longitudinal melanonychia is t...
